Quirino State University (“QSU”, “We”, “Us” or “Our”) respects and values your right to data privacy. Thus, QSU is committed to comply with the Data Privacy Act of 2012 (DPA), its Implementing Rules and Regulations, and to all relevant issuances of the National Privacy Commission (NPC) to ensure personal data protection.

This notice explains the practice of the University in processing your personal data from collection to disposal and to obtain consent from you. Please note that this privacy notice applies to the handling of your personal data as an applicant and employee of the University.

INFORMATION COLLECTION, USE AND PURPOSE OF PROCESSING

The university processespersonal data about you in order to perform its mandate as an educational institution from your application for employment to the time your employment ends. The collection may be done through the forms/documents you submitted to us either in written or electronic format. In some instances, your images and videos are being captured by the Closed Circuit Television (CCTV) cameras installed in the university.

Applicant. When you apply for employment, the following documents will be collected: Curriculum Vitae/Resume, Transcript of Records, Diploma, Certificate of Seminars and Training attended, Eligibility and Licenses, and other supporting documents.

Employee. When you are hired, we process your personal data as specified in the Personal Data Sheet such as Name, birthdate, birthplace, sex, civil status, citizenship, address (permanent and residential), height, weight, blood type, Government issued ID Numbers (Pag-ibig, GSIS, Philhealth, SSS, TIN and Agency Employee No.), contact details (mobile number, telephone and email address), Family Background, Educational Background, Civil Service Eligibility/Licenses, Work Experiences, Voluntary Works and/or Involvement in Civic/Non-Government/People/Voluntary Organizations, Training Skills Development, Special Skills and Hobbies, Non-Academic Distinctions/Recognition, Membership/Affiliations in Association/Organization, References, Photo, Signature, Payroll data and tax details, benefits, promotions, performance evaluation, leave of absences, disciplinary records, Health and medical records. The following documents are also collected from you: Transcript of Records and Diploma, Certificate of Training and Skill Development programs you have attended, Eligibility and/or licenses, PSA Birth Certificate and/or PSA Marriage Contract is married, Health Records and Psychological Exam Results and SALN.

DISCLOSURE/SHARING

The University may disclose your personal data to third parties with your consent or when required or allowed by law. Such disclosure is subject to procedure to ensure the identity and legitimacy of the third party. The University may disclose or share or transfer your personal data, but not limited to the following:

  • Government Agencies such as Commission on Higher Education (CHED), Civil Service Commission (CSC), Payroll Bank Depository (Landbank); GSIS/SSS, BIR, PhilHealth, Pag-ibig;
  • Accrediting agencies such as AACUP for accreditation purposes; and
  • Companies/agencies verifying the employment records of former employee/s

STORAGE

Your personal information either in paper or electronic format is stored and transmitted securely. Access is limited to authorized personnel with utmost confidentiality. Rest assured that the University is implementing reasonable and appropriate organizational, physical, and technical security measures to ensure protection of your personal and sensitive data.

ACCESS

You have the right to access, update or correct your personal and sensitive information as per requested by the concerned party. In accessing your own personal information, the University will require you to present your University ID or other documents to confirm your identity. In case of the absence of the concerned party, the University will require him/her to provide a letter of consent and present valid ID’s of both the concerned party and representative.

 

RETENTION AND DISPOSAL/ DESTRUCTION

The University will retain your personal data for a period of time allowed by law and/or University policy and destroyed securely when no longer needed.
